The Healthcare Poland Foundation’s Coordinated Care Center is a knowledge and coordination hub where the experience of hospitals, specialist clinics, long-term care providers and the patient’s home environment is meant to come together into one coherent chain of health and social value.

Why coordinated care

Coordinated care in Poland today is delivered mainly at the level of primary care, under the POZ PLUS model rolled out since October 2022. After its first eighteen months, the model covered roughly 32–33% of primary-care providers. What is still missing is an organization integrating the full value chain: from primary care, through specialist outpatient care and hospital treatment, to rehabilitation, long-term care and support for the patient in their own living environment.

In practice, institutional fragmentation means a loss of information continuity across primary care, specialist care, hospitals and the patient’s home; the absence of shared health-value metrics spanning the whole treatment episode and the patient’s return to functioning; inefficient allocation of staff and infrastructure; and weak protection of the patient’s rights as a beneficiary of coordinated care.

Mission

The Center’s aim is to integrate the full care value chain — from prevention and primary care, through specialist outpatient care, to hospitalization, rehabilitation, long-term care and support in the patient’s living environment, including social assistance and personal assistance — so as to systematically raise health and social value for the citizen and protect their rights as a beneficiary of an integrated care system.

Institutional cooperation

Polish Federation of Hospitals (PFSz) — a network of more than 250 hospitals and roughly 150,000 beds, giving the Center standing within the hospital community and a platform for social dialogue.

Association of Coordinated Care Employers (ZPOK) — an initiative co-founded by the Healthcare Poland Foundation together with the Polish Federation of Hospitals, the National Chamber of Long-Term Care Homes and the Polish Home Care Association. It is the first employers’ organization in Poland integrating the full coordinated-care value chain — complementary to, not competing with, the primary-care-centered SPOIWO association.
