The twelve value streams describe what Healthcare Poland Foundation delivers. The self-learning loop describes why what we deliver should be better tomorrow than it is today — and turns that into an operational commitment rather than a declaration. It is not part of the offer and cannot be ordered as a service. It is how we grow together with our partners.
The mechanism
Every signal goes into a single register: a recurring question, an audit finding, a gap in the resource catalogue, an incident, a regulatory change, training feedback, a mediation outcome, a pilot observation. When the same signal appears for the third time, the system marks it as a missing resource. From there, only three outcomes are possible: a resource, a task, or a refusal with stated reasons. No signal can remain undecided — that is the difference between a loop and a suggestion box.
Every cycle ends with a decision recorded together with its reasons.
Why it is part of the system, not a service
An organisation that reports a problem receives a tool, not a thank-you note. The experience of one facility becomes a standard available to the others — a benefit no single hospital can create alone, because it has no access to the experience of the rest. That is why the loop is never sold separately: it is built into every stream and works for the whole partner network.
Who we grow with
Polish Hospital Federation
The Foundation is a supporting member of PFSz — a network of more than 250 directly affiliated hospitals and, together with umbrella organisations, around 520 entities. Signals come from this practice; pilots run in this network.
Facilities and stream partners
Hospitals, local governments, universities, technology providers and international partners using our streams bring their own experience into the loop and receive jointly developed solutions in return.
Permanent sources feeding the loop:
- audits by the Quality, Audit and Certification Centre (CAC HCPL),
- mediations at the Mediation, Conciliation and Arbitration Centre (CMA),
- training at the HCPL Academy (EDU HCPL),
- pilots at the Innovation Centre (CI HCPL),
- incidents reported in the CyberC4HE Coalition.
The link to Just Culture
An adverse event, the way it was handled and the lesson drawn from it are three different things. The loop deals with the third: it turns the event into learning material, with the reasons for each decision recorded — including when the decision is a refusal. A refusal without reasons closes the loop after a single cycle.
Operating conditions
- a single signal register with a named person responsible for it,
- a response deadline for whoever raised the signal,
- permanent input sources from every stream,
- a quarterly review of what has actually come out of the signals.
