Personal Data Processing in Partnership with Healthcare Poland Foundation

  1. Data Collection and Purpose:
    • Representatives of Supporting Entities: We collect names, contact numbers, positions, and email addresses of individuals authorized to represent entities supporting the Healthcare Poland Foundation. This data, included in the membership declaration, is used for activities related to being a partner of the Foundation, particularly for executing rights and obligations from the Foundation’s statute and for sending newsletters with HCPL updates, in accordance with Article 6(1)(f) of the GDPR.
    • Individual Partners: Names, contact numbers, email addresses, tax identification numbers (NIP), positions, and addresses of individual partners are collected for contract fulfillment under Article 6(1)(b) or (c) of the GDPR. This includes settlements as per legal requirements and member rights and obligations, as stated in the membership declaration.
    • Management of Medical Entity Partners: Names, positions, business email addresses, and business phone numbers of the management of medical entities partnering with the Foundation are collected either from the membership declaration or public sources, including the National Court Register (KRS) and the medical facility’s website. This data is used for activities related to being a partner of the Foundation, particularly for executing rights and obligations from the Foundation’s statute and for sending newsletters with HCPL updates.
  2. International Data Transfer: Personal data may be transferred to third countries.
  3. Data Disclosure: Personal data may be disclosed to entities providing technical and administrative support services to HCPL.
  4. Data Retention: Personal data will be stored for the duration of the partnership with HCPL and for the period in which claims between HCPL and its member/partner may become statute-barred.
  5. Data Provision Requirement: Providing the personal data mentioned in points 1a and 1b is necessary for contract execution and joining HCPL. Refusal to provide this data may prevent membership or other forms of cooperation with the Healthcare Poland Foundation.

    • Access your personal data;
    • Request correction of your data;
    • Request deletion of your data;
    • Request restriction of processing your data;
    • Object to data processing;
    • Request the transfer of your personal data.
  7. Complaints: You have the right to lodge a complaint with the supervisory authority, i.e., the President of the Personal Data Protection Office in Warsaw, regarding the processing of your personal data by HCPL.
  8. Automated Decision Making: Your data will not be used for automated decision-making, including profiling.
